In this key role, you will serve as the primary liaison between hospitals, referral sources, residents, and families, ensuring a seamless admissions process while helping drive occupancy growth and exceptional customer service. Key Responsibilities * Coordinate and facilitate the admissions process from referral through admission. * Collaborate with hospitals, physicians, case managers, and referral sources to ensure smooth resident transitions. * Develop and maintain strong relationships with referral partners and community organizations. * Support marketing and outreach efforts by attending networking events and community functions. * Educate prospective residents and families regarding services, policies, visitor guidelines, and admission procedures. * Verify insurance coverage, financial information, and authorization requirements prior to admission. * Maintain working knowledge of Medicare, Medicaid, Managed Care, and payer-specific guidelines. * Monitor admissions activity and prepare monthly reports on admissions trends and census growth. * Identify operational barriers affecting admissions and work with leadership to implement solutions. * Communicate admission details effectively with interdisciplinary team members to ensure continuity of care. **Qualifications** * Minimum of 2 years of admissions experience in a Skilled Nursing Facility (SNF) required. * Strong understanding of Medicare, Medicaid, Managed Care, PDPM, HMO guidelines, and RUGS II \& III. * Experience working with Electronic Medical Record (EMR) systems. * Knowledge of CMS regulations and Department of Health (DOH) requirements. * Excellent communication, organizational, and relationship-building skills. * Ability to multitask and work effectively in a fast-paced healthcare environment. **Benefits** * Paid Time Off (PTO) * Medical Insurance * Dental Insurance * Employee Recognition Programs * Company Perks * Supportive and Collaborative Work Environment * Opportunities for Professional Growth and Development Join a team dedicated to providing exceptional care and service to our residents and their families.
