We are seeking an experienced Physician Assistant (PA-C) to join our growing Mobile Primary Care team serving seniors in home-based care, assisted living (ALF), skilled nursing facilities (SNF), and senior living communities. This is not a traditional clinic role. This is community-based, field-based medicine focused on geriatrics, chronic disease management, dementia care, and value-based care delivery.
